Please use the forms and fees information below to provide us with the required information for services in 2023.
If you are completing a form for a funeral taking place outside of church premises, officiated by a Retired or SSM minister please use the PF1 - Funerals (2023) form and put "DBF" in the Parish Name field, you will then need to put the mileage on the PF1 and PF4 form in order for the Finance office to make payment. If the service is officiated by a stipendiary minister licensed to a Parish please put the Parish name on the form and mileage claims will be paid to the parish directly.
In February 2023 the General Synod approved the parochial Fees (Amending) Order 2023, which capped increases to 5% in 2023 and 2024 – this means fees will reduce from those introduced on 1 January 2023. The revised fees are effective from 1 March 2023 and apply to all services/events from that date.
The PF1 form below will automatically identify the correct fee but for PF2, PF3 and PF4s, please select the correct form based on the date of the service.
